NextLevel Health Partners, Inc., and PatientPing to Improve Member Care and Coordination in Illinois

BOSTON— (January 17, 2019) – PatientPing, the nation’s most comprehensive care coordination network, today announced that it has partnered with NextLevel Health, a Medicaid managed care company in Illinois serving Medicaid beneficiaries and those participating in the Managed Long-Term Services and Supports program.

By partnering with PatientPing, NextLevel Health will receive real-time information on member events, including ED presentations and discharges, to further identify and engage its high-risk Medicaid members and ensure connectivity to community based care post-discharge. PatientPing will also provide NextLevel Health with critical information on its members, including updated phone numbers, helping to increase engagement for their Unable to Contact (UTC) populations. NextLevel Health will also have access to PatientPing’s existing Illinois network, which includes key health systems such as Advocate and AMITA Health.

Through the partnership, PatientPing will receive important data on all of NextLevel Health’s  57,000+ members receiving care across hundreds of hospitals, behavioral health facilities, Federal Qualified Health Centers, PCPs, specialists and ancillary services throughout Cook County.

NextLevel Health is a participant in the Integrated Health Home program which aims to better coordinate care for high complexity patients and achieve success with chronic care management, transitional care management and health promotion. Real-time alerts are sent directly to the team member responsible for patients enrolled in the program. “Safe hospital to home transitions are contingent on collaboration amongst healthcare providers,” said Cheryl Whitaker, MD, MPH, FACP, CEO of NextLevel Health. “This partnership will enable us to strengthen our care coordination efforts so that transition to home and back into the community based primary care loop has a chance to occur.”
