PatientPing and VITL Working Together Successfully to Advance Patient Care Coordination and Further Interoperability Efforts on a Statewide Level, with Participating Organizations Reaping Immediate Benefits from Adopting PatientPing
Boston, MA (September 28, 2016) – PatientPing, a health technology company that connects providers to seamlessly coordinate patient care, announced today that within a few short months, over 400 provider sites across New England have been notified through PatientPing that their patients have been seen at Vermont hospitals, thus allowing for the collaboration of providers across state lines. In the six months since go-live, and using an information stream provided by Vermont Information Technology Leaders, Inc., PatientPing has logged over 62,000 notifications on approximately 12,000 Vermonters, helping providers improve the quality and safety of patient care. This milestone represents a huge step forward in achieving higher quality care through better patient care coordination in the state.

PatientPing enhances medical information transparency between health care providers. Upon joining PatientPing, providers receive “Pings,” or real-time notifications, whenever their patients receive care at a wide range of facilities. Nicole Keaty, RN, Telehealth Manager at Central Vermont Home Health & Hospice, explains, “PatientPing gives us the ability to quickly follow up with a patient when they are readmitted to a hospital. I believe this greatly enhances continuity of care, and patients are really appreciative when we reach out.”
In one patient case at Central Vermont Home Health & Hospice, an 80-year-old male suffering from heart failure had been admitted and discharged from the hospital repeatedly over the course of one month. Keaty and her team received the notifications, and were able to follow up with the patient to check in on him. They were also able to help coordinate with his other health services to determine what resources he needed in order to stay healthy at home, such as regular phone check-ins and educational materials.
Keaty adds, “Managing chronically ill patients takes a village, and it’s important that their care teams are all on the same page. PatientPing helps us do just that.”
