Event Recap: PatientPing and the Connecticut Hospital Association Roll Out Stories to CT Hospitals
Last week, PatientPing hosted an event at the Connecticut Hospital Association (CHA) offices to roll out its Stories product to hospitals across the state. Stories provide real-time patient context at the point of care so that providers can more accurately make treatment decisions. Stories include important information such as patient care team contact information, prior visit history, diagnostic information, and much more. Currently, real-time notifications (Pings) and patient context (Stories) are being utilized by PatientPing’s ACO, SNF, and home healthcare partners across the state of Connecticut. The event last week marked a significant milestone in which hospital stakeholders joined the PatientPing network, further increasing care coordination efforts across the entire continuum of care.
The morning began with breakfast and opening remarks from John Brady, CHA’s CFO, and Sarah Wigman, PatientPing’s Connecticut Operations Manager. John and Sarah welcomed attendees and reviewed care coordination progress in the state since PatientPing’s partnership with CHA went live. In the past 12 months, PatientPing has sent providers over 1.2 million Pings on over 200,000 patients across 17 states.
Following the opening remarks, Caroline Segovia-Marquez, RN, BSN, MBA, ACM-RN, CMAC, CPHM, Director of Case Management at Saint Francis Hospital and Medical Center (SFHMC) & Johnson Memorial Hospital presented to the group. She discussed how her team has been using PatientPing’s Stories. When a patient presents to their ED, the team at SFHMC uses Stories to begin an assessment before even speaking with the patient. Prior to using PatientPing, the team at the hospital had no insight into the patient’s prior utilization. They now use Stories as the ‘source of truth’ for the patients’ information. In Q1 alone, SFHMC has placed 29 patients to non-acute facilities and services, successfully avoiding readmissions.
